Description and Service

Description (Service)	WrapAround Program supports families as they build a team of formal and informal supports based on the strengths and skills of trusted family members, friends, and the community in order to develop and achieve goals meaningful to them provides a planning process that guides families as they move towards their chosen goals a voluntary process that includes monthly support team meetings planning process takes approximately twelve to eighteen months. three certified WrapAround Facilitators, including a Facilitator able to offer an Indigenous approach for those families connecting with their Indigenous culture, are available Facilitators are certified in the High-Fidelity Model of WrapAround and are a part of the Wrap Canada Training Institute, members of WrapAround of Ontario and Wrap Canada
Eligibility	Families involved with Simcoe Muskoka Family Connexions First Nations, Inuit and Métis families
Application	Referral through Simcoe Muskoka Family Connexions, with the exception of First Nations, Inuit and Métis families, who may self-refer to Catulpa Community Support Services directly or ask to be referred through an involved First Nations, Metis, and Inuit (FNMI) Facilitator
