Description (Service) Assessment and intervention services for infants and preschool children who are blind or low vision as diagnosed by an ophthalmologist * Child and Family Intervention Services and Childcare Consultation available * provides a Family Support Worker to families of eligible children where indicated * will assist families to access service when low vision/blindness is suspected but accessing a diagnosis is difficult
Eligibility Ages: 4 year(s) and under 
 
Screening - infants * services up to junior kindergarten entry maximum for identified children eligible for specific supports * diagnosis blind or low vision
Application Referral - Families * Referral - Physicians * Referral - Optometrists * Referral - Early Interventionists (with written diagnosis from an ophthalmologist) * intake worker gathers basic information and mails a background information package for child's family to fill out
