Description and Service

Description (Service) Patient-centred education and support to individuals living with Type 1 Diabetes, Type 2 Diabetes, Prediabetes or Gestational Diabetes
  • Consists of a team of Endocrinologists, Registered Dietitians (Certified Diabetes Educators) and Registered Nurses (Certified Diabetes Educators)
  • Provide information and education around healthy eating, exercise, monitoring blood sugar levels, role of medications and insulin we well as other lifestyle factors
  • Insulin pump program site
Eligibility Adults 18 years and older with Type 1, Type 2 or Gestational Diabetes as well as those with Prediabetes
Application
  • Call for an appointment, self-referrals are accepted (please find self-referral form on our website, Google www.bchc.ca)
  • Endocrinologist Appointment: physician referral is required
