Former Name Ontario Long -Term Care Home for the Deaf
Description (Service) 64-bed long-term care facility for adults who are deaf, deaf-blind, hard of hearing and hearing * has a multipurpose room for use by both residents and the Deaf Community * residence is the only Long-Term Care facility for people who are deaf, deaf-blind and hard of hearing in Canada
Eligibility All adults who need long-term care * not restricted to adults who are deaf, deaf-blind and hard of hearing
Application Contact North Simcoe Muskoka Local Health Integration Network (NSM LHIN) - Home and Community Care at 1-888-721-2222 or 705-721-8010 * Home and Community Care Coordinators will assess the needs of applicants for eligibility and needs assessment
Languages English ; American Sign Language (ASL) - Reception, Interpretation, Programs
Fees Service - Rates set by the Ontario Ministry of Health and Long-Term Care * subsidies for basic rates may be available for eligible applicants
Volunteer Description * volunteers will be recruited from hearing and deaf communities within the Simcoe County area and can be placed in all departments
