Former Name Fall Assessments ; Falls Assessments
Description (Service) Community consultation service targeting frail seniors with geriatric syndromes requiring comprehensive geriatric assessment by an inter-professional team with specialized knowledge and skills regarding the care of frail seniors * focus on individuals with falls/fear of falling * team includes nurses, physiotherapists and occupational therapists * services include home visits and screening clinics, with Geriatricians supporting Specialized Assessment Clinics in Barrie and Orillia
Eligibility Frail Older Adults (Clinical Frailty Scale 4-6) residing in community dwellings: 
* seen by health provider with evidence/documentation of falls assessment AND meeting 1 or more of the following criteria: 
* presenting with a fall in a NSM Emergency Department and being discharged home 
* OR being referred by a NSM fracture clinic 
* OR requiring falls assessment and intervention by a specialized interprofessional team in an ambulatory clinic setting
Application Physician or Nurse Practitioner referral required for Geriatrician /Specialized Assessment Clinic
