Description and Service

Description (Service) Adults who have a complex functional deficit requiring assessment, education and/or therapeutic intervention by two or more health care professionals (Occupational Therapy, Physiotherapy, Speech Language Pathology).
Eligibility
  • Adults who have a functional deficit requiring assessment, education and/or therapeutic intervention by two or more health care professionals
  • Does not accept patients that require only one of the listed therapies
  • 19 years and older, however exceptions may be made with appropriate diagnoses
  • Application All referrals must be initiated and signed by a physician, forms available online.

    Fees None - All RDH services are covered by OHIP; Transportation and associated costs are the responsibility of the patient/family.
