Since June 30, 2015 Hospice Simcoe has been offering 24 hour end-of-life compassionate palliative care at our residence.

Office: Mon-Fri 9 am-5 pm

10 bed residence at Hospice Simcoe provides care to individuals who are living with a palliative diagnosis, supporting them and their families with dignity and comfort in a home-like environment.

• staffed twenty-four hours a day by a professional palliative care team and trained compassionate volunteers
• spacious private rooms enable family members to stay together
• pain and symptom management

Children (4 yrs and older), Youth, Adults; Older Adults

Assessment is required by Ontario Health atHome.

• Referral request must go to Ontario Health atHome, phone: 705-721-8010
• The request can come from a physician, client or substitute decision maker for the client
• Ontario Health atHome will complete an assessment and referral prior to admission to Hospice Simcoe,
  admission is based on a triage assessment tool that identifies the person most needing an admission
