Adult day program * social recreational and therapeutic activities for seniors and adults who are frail, cognitively impaired, and/or isolated, and/or physically challenged * helps to support family caregivers who provide day-to-day care * noon meal and snack are provided * assistance to make transportation arrangements

Older adults, adults disabilities or cognitive impairments who need physical, social or emotional supports * Resident of Simcoe County
North Simcoe Muskoka Local Health Integration Network (NSM LHIN) Home and Community Care Community Care Coordinators assess the needs of applicants
