Description (Service)	Partnership between the March of Dimes Canada and Mackenzie Health provides support to individuals who live with the effects of an acquired brain injury Team includes Behaviour Consultants, Case Managers and Rehabilitation Workers Services include: case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them individualized service/treatment plan, developed with recommendations and community support to promote the acquisitions of skills and strategies plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community
Eligibility	Age Requirements: Ages: 16 year(s) and up Youth, Young Adults, Adults, 16yrs and up with brain injury, reside in Simcoe County, Muskoka or York Region
Application	Referral - self, professional or other (consent is required to make referral)
Languages	English; French - Reception, Website, Interpretation, Programs, interpreters provided where possible
