Description (Service) Community based support to individuals and their families who are living with the ongoing effects of an acquired brain injury (ABI) * rehabilitation services are based on each person's specific needs as identified by the individual and their support network * program components are delivered through one on one support or group activities and may include: 
* Brain Injury Education  
* Family Support and Education  
* Cognitive Rehabilitation  
* Community Integration  
* Resource and Referral service  
Accredited by Accreditation Canada and a member of the North Simcoe Muskoka Community Support Services Collaborative (NSMCSSC) and the North Simcoe Muskoka Acquired Brain Injury Collaborative (NSMABIC)
Eligibility Ages: 16 year(s) and up 
Residents of Simcoe County, District of Muskoka, 16 years or older, whose primary diagnosis is acquired brain injury and who are living with the on-going effects of an acquired brain injury
